You will need your loved one's information, as well as:
A valid state ID, driver's license, or passport.
A mobile device with a camera to complete a quick identity check.

A complete PDF or clear photos of all pages of your notarized power of attorney document.
Note: If your power of attorney document is more than two years old, you will also need a notarized Affidavit of Full Force and Effect. (The exact name of this form varies from state to state, but its purpose is to declare that the power of attorney is still in effect.)
Please note: Your power of attorney document must:
- Explicitly grant the you power of attorney over your loved one
- Be notarized
- Have witness signatures if required by the state
